The Objective of the Fund

Grants of up to £10,000 available for Primary Care Networks (PCN’s) in Coventry, Solihull and Warwickshire to set up a Lifestyle Clinic.

What is a Lifestyle Clinic

The Lifestyle Clinic model has been successfully developed by Leamington PCN. It’s a group session with around 5-10 pateints. Clinics run over 3 weeks for an hour a week, followed by a 30 minute low intensity physical activity session.

Each week has a different ‘lifestyle’ focus, for example:

Week 1: Nutrition

Week 2: Changing Behaviours and linking with the community

Week 3: Being Active.

As the funding is being provided through Think Active, supporting patients to be active and delivering key messages around being physically active must run throughout the sessions

Each clinic session should end with a low intensity physical activity eg  seated exercise or walking session such as WalkTalkWalk

The clinics can be run by GPs, practice nurses, dieticians, social prescribers & health & wellbeing coaches.

PCN’s are encouraged to develop a model which best reflects local priorities and networks.


Eligibility criteria and expectations

The Lifestyle clinic should be established on behalf of all the practices within the PCN

It is expected that the PCN delivers one 3-week clinic each month for 12 months.  Alternative models will be considered.

A lead clinician should be identified who will be responsible for setting up the clinic – although different colleagues can be brought in to deliver the group sessions.

A lead organisation must be identified to receive the funds, this could be a GP practice within the PCN, GP Federation or another similar organisation.

All practices within the PCN should be supportive of the clinic and must be willing to sign up to the RCGP Active Practice Charter, this is simple to do on the following link


Benefits of establishing a lifestyle Clinic

There are a range of benefits to the PCN and GP Practices in establishing a Lifestyle Clinic, these include:

  • Improve the health and wellbeing of patients and morale of practice staff
  • Help hit social prescribing referral targets quickly
  • Utilise Additional Roles Reimbursement Scheme staff effectively (Health & wellbeing coaches, Dieticians, Social prescribers & pharmacists)
  • Foster strong relationships between PCN and practice staff members as they work together on the project
  • Reducing the burden of disease & medication
  • Raise awareness amongst the local community of the range of services that practices provide
  • Supporting community & public health projects like this can help achieve high CQC ratings as well as improved patient satisfaction scores
  • Connect & signpost more effectively with local charities and community groups.

What can the funding be spent on

Up to £10,000 is available to set up and run the clinic for 12 months.  This can be used to pay for the clinician’s time to set up, deliver and run the clinic as well as any resources or training which may be required.


Monitoring and evaluation

A simple monitoring and evaluation framework will be developed which the PCN will be asked to report on.  This will include patients’ stories & quantitative data e.g. weight, BMI, blood pressure & hba1c. This will depend on what the focus of your clinic is.



Monthly support sessions will be available with Dr Hussain Al-Zubaidi, founder of the Leamington PCN Lifestyle Clinic.

In addition, the Think Active Team will be able to support practices to sign up to the RCGP Active Practice Charter, make links to community activities and signpost to training and resources.

The funding can also be used to pay for staff to complete additional Lifestyle Qualifications or example The British Society of Lifestyle Medication Certification in Lifestyle Medicine or the  Inspired Medics Lifestyle Medicine Course
